Eligibility & Funding Policy
To ensure we can help those most in need, please review our eligibility criteria and program policies before applying. This will help you understand if our support is the right fit for your family.
Eligibility Criteria For Patients
Children & teens 0–18 yrs; application must be submitted by a parent or guardian.
Documented financial hardship (verified by W‑2, proof of benefits, etc.).
No active insurance coverage for therapy, or a written denial of coverage from your insurer.
Not currently receiving funding for therapy from another organization.
Family agrees to our 5‑step intake, monitoring & follow‑up process.
Parent or guardian acknowledges and agrees to all Bereishis Foundation policies via a checkbox in the application form.
We Cannot Fund If…
Applicant is 19 years of age or older.
Insurance coverage for therapy is available.
Another communal fund can fully cover the therapy need.
Sessions occurred before Bereishis Foundation approval.
Financial documentation or references are missing or unverified.
Therapist lacks a valid U.S. license.

Support Process
1. Application: Submit application form with parent info, W‑2, therapist details & reference via our online application.
2. Review & Reference Check (1–3 days): We verify references, therapist credentials & alternate funding options.
3. Initial Approval: First 5 sessions approved. Email confirmation sent to family & therapist.
4. 5‑Week Check‑In: Parent completes progress form via online check-in form; financial need reassessed, including any changes in insurance or partial funding contributions.
5. Reassessment / Additional Blocks: Funding may extend in 5‑session blocks based on progress, budget, and continued need (typically limited to 12 months total, with exceptions reviewed on a case-by-case basis).
Attendance & Funding Continuity Policy
Inactivity: Funding is discontinued if a patient no-shows or cancels two consecutive sessions without rescheduling within 48 hours.
Emergency Absence: Families may request up to 4 weeks excused for emergencies (medical, natural disaster) by submitting documentation to Connect@BereishisFoundation.org.
Rescheduling: Must notify therapist/Bereishis and reschedule within 48 hours of a missed session. Max 2 reschedule requests per 5-session block.
Notification: 1 missed session triggers a warning email. 2 consecutive missed sessions trigger a final discontinuation email.
Appeals & Reinstatement: Appeals accepted within 10 business days. Families may reapply after a 30-day waiting period with a therapist letter confirming commitment.
Session Frequency Requirements
Patients must attend at least one therapy session per week to maintain funding eligibility.
Requests for adjusted frequencies (e.g., biweekly) must be submitted by the therapist with clinical rationale to Connect@BereishisFoundation.org (reviewed within 5 business days).
Therapist Payment Policy
Before Funding: Therapist completes online application; session rate approved in writing. Bereishis covers the remainder if parents partially contribute.
Ongoing Requirements: Report completed/missed/rescheduled sessions and format (in-person/telehealth) within 48 hours to Connect@BereishisFoundation.org.
Invoicing: Invoice after each session (no pre-payment). Invoices sent over 7 days after completion will not be paid. No-shows are not reimbursed.
Virtual Sessions: Telehealth covered at the same rate. Must use HIPAA-compliant platforms and confirm compliance in session reports.
Collateral Sessions: Parent/guardian sessions covered if billed at the child's approved rate, adhering to all standard invoicing and attendance rules.
Substitution & Adjustments: Notify Bereishis within 48 hours if a therapist becomes unavailable. Changes in family finances (e.g., new insurance) must be reported during the 5-week check-in.
Administrative Policies
Late Submissions: Forms due within 5 business days (3-day grace period). Late submissions may trigger a funding review. Appeals allowed within 5 days of notice.
Incomplete Applications: 10-business-day window to submit missing items before deferral to the waitlist.
Parent Communication: Parents must respond to Bereishis emails within 5 business days to maintain active funding status.
Dispute Resolution: Conflicts must be submitted in writing to Connect@BereishisFoundation.org within 10 business days for representative review and potential mediation.
Waitlist, Confidentiality & Ethics
Waitlist: If the budget is fully allocated, eligible families are placed on a waitlist and notified when funds reopen.
Confidentiality: All application data is strictly confidential. References are contacted only to confirm need; clinical details remain private.
